Privacy

A patient's right to privacy is very important to me. All medical records will be held in strict confidence. I will not release any information without written permission from you or a responsible person for you.

Insurance

I am a preferred provider with most insurance companies and provide primary care for teens and adults. I suggest you call your insurance company and verify your coverage as you are ultimately responsible for any unpaid bill. You are certainly welcome to see me if I am not preferred by your insurance company. However, you will be responsible for the difference that your insurance company won't cover. I will also see you if you've been in a motor vehicle accident (MVA) as long as you have a PIP (personal injury protection) number from the automobile insurance company. Otherwise you will be expected to pay cash at the time of service and I will give you a bill of service. Your health insurance company cannot be charged for a visit related to an MVA.

I have an offsite biller who handles all insurance claims. Sandy Harrison can be reached at H&H Medical Billing at
425-259-6200 Tuesday through Friday.

For those of you without insurance, I charge 20% less for an office visit since you will be paying at time of service. Feel free to call the office if you need more information.

Forms

If you are a new patient please fill out the patient forms if they were mailed to you or arrive at least 15 minutes early to allow time to fill them out. Please bring your forms to your first appointment or fax them to our clinic at 206.525.8014.
